Please fill the form below to consent that your child is eligible to participate in an event/activity organized by Mineral Baptist Church's (MBC) Student Ministry, requiring transportation to a location away from church premises. The activity will take place under the supervision of MBC Student Ministry leaders approved, verified, and background checked by MBC.
By completing this form, you agree that you understand the nature of this activity and the associated risks, and in case of an accident in transit or on location, that you do not hold Mineral Baptist Church or adult chaperones responsible for injuries and understand that your insurance will cover such circumstances should they occur, as the Primary Insurance carrier. You also give your permission for the group leader or an adult leader to authorize medical treatment for my child at the nearest emergency care facility, should an injury or illness occur, after contact has been made with you. You also understand that certain behavioral guidelines have been placed upon your youth/child during this trip. You understand that he/she are expected to adhere to these guidelines. In addition, you understand that failure to follow these guidelines could result in him/her being sent home at your own expense.

I give permission for my child to be photographed or recorded during the mission trip and for these images to be used by the church for ministry and promotional purposes.
